SFC Begins Offering Case Management!

Starting this week, St. Francis Center is excited to officially launch case management as part of our Resource Services for guests! This new component of our program offerings will expand our current Resource Services, which include referrals for health care services, shelter for families and individuals, and assistance obtaining IDs for jobs and social security.

SFC’s Resource Specialist, Caitlin Powers (Kitty), will schedule weekly one-on-one meetings with about five to 10 guests in the beginning and gradually increase the number of guests in case management. SFC is now a part of a cohort of nonprofits that coordinate services for homeless clients through the Los Angeles Homeless Services Authority’s (LAHSA) Homeless Management System (HMIS). HMIS streamlines the Resource Services SFC provides and delivers comprehensive care more effectively overall. Kitty will also receive training on the Coordinated Entry System (CES), which is a means to quickly and efficiently match people to available housing resources and services that best fit their needs.

“It is amazing to be part of the new Case Management program at SFC and I am looking forward to seeing the way our guests work to achieve their goals. Working with our guests every day is a special privilege that I have as Resource Specialist and I feel extremely lucky to be able to engage even more with those who are beginning case management with us,” Kitty said.
